This form must be completed for every new coach to the Club. Potential Coach is to fill in names and information of references. The Club will contact the references as per the questions on the form. |
Player Medical-Contact Information Form
|
This form is intended for Coaches to collect contact and medical information on their players. Print and ask players/parents to fill in (as required for medical conditions the coach should be aware of). Forms must be destroyed at the end of each season.
